    We will kick off the collaborative with an in-depth exploration of the LTCSP and the foundations of regulatory compliance. This session will guide participants through the critical tools and resources needed to prepare for and navigate surveys, emphasizing the importance of a culture of compliance and readiness

    February 27: Survey Strategy and Risk Evaluation 

    Learn how to use your facility specific Survey Strategy Worksheet to identify and address risks. This session will cover how to evaluate trends, understand the resident interview process, and incorporate these strategies into daily operations for sustained compliance

    March 27:  LTCSP Mandatory Task Observations 

    Dive into the LTCSP mandatory tasks, focusing on observation processes across departments. Learn how to conduct effective observations and use findings to enhance compliance and quality improvement

    April 24: LTCSP Record Review Process 

    Gain a comprehensive understanding of the LTCSP record review process. This session will help participants integrate record reviews into daily compliance activities to sustain survey readiness. Understand the Interdisciplinary Team (IDT) role in mastering the LTCSP record review process to strengthen compliance. Learn how to involve clinical, dining, environmental services, and other teams in department reviews and resident record reviews, identifying compliance risks, and implementing sustainable practices.     May 22:  Managing the Survey Process 

    Prepare your team to manage the survey process confidently. This session will provide best practices for planning, assigning roles, and utilizing real-time tools to ensure compliance during surveys

    June 26: Post-Survey Process and Sustainability 

    Wrap up the collaborative by mastering the post-survey process, including development of a Plan of Correction (POC). Learn how to incorporate POC processes into year-round compliance efforts and understand the implications of Civil Money Penalties (CMPs)

    The 2025 QIP Collaborative comes at a critical time as Ohio Medicaid reimbursement undergoes significant changes.

    The 2025 QIP Collaborative comes at a critical time as Ohio Medicaid reimbursement undergoes significant changes. These shifts will have a direct impact on nursing home providers, and this collaborative is designed to ensure that participants are fully informed and prepared to navigate these changes. Throughout the series, providers will receive timely updates on these developments, helping them understand how the evolving landscape will affect both their quality measures and reimbursement. Additionally, LeadingAge Ohio is excited to share that participants will meet the new Ohio Administrative Code language for the Nursing Home Quality Initiative found in Chapter 173-60 of the OAC, which became effective in November, by participating in this collaborative.

    This collaborative will build on the evidence-based practices from previous years while focusing on a more interactive, group-centered approach. Participants will work together in groups based on their selected quality measure, applying evidence-based strategies and working through each stage of the Performance Improvement Process (PIP) with guidance and support. This collaborative will foster a peer-driven learning environment, allowing participants to share insights, refine their approaches, and track progress. Facility-specific reporting will be provided, ensuring providers can make data-driven decisions while staying informed on updates to Ohio Medicaid reimbursement and QIP changes.

    Education hours are available with each session.

    This session will examine the Quality Incentive Payment framework for the fiscal year 2025 budget, analyze the outcomes and insights derived from QIP Collaboratives, and reflect on valuable lessons learned. Participants will learn how they will work together in groups based on their selected quality measure, applying evidence-based strategies, and working through each stage of the Performance Improvement Process (PIP) with guidance, tools, and support.

    February 20: Data Collection and Analysis: Leveraging Frozen Quality Measures

    This session will begin with a review of the newly available data for the previously frozen quality measures, providing participants with updated information and actionable insights. Participants will then learn how to integrate these data-driven approaches into their team dynamics to drive continuous improvement. We will emphasize the importance of fostering a culture of collaboration and accountability within healthcare teams. By applying the lessons learned from the QIP Collaborative group work, participants will gain strategies to create an environment of trust and mutual responsibility in their nursing home settings, ensuring accountability across all levels of care.

    March 20: Implementing Evidence-Based Practices for Sustainable Improvement

    This session will focus on the implementation of evidence-based practices as a key driver for achieving sustainable improvements in nursing home care. Participants will explore proven interventions and strategies that align with their facility's quality measure goals. By learning how to effectively integrate these practices into daily operations, leaders will be equipped to ensure long-term success and continuous improvement. The session will also emphasize how to monitor and adjust these practices to ensure they remain relevant and impactful over time.

    April 17: Collaborative Problem-Solving in Performance Improvement Process Implementation 

    This session will explore how leadership can facilitate collaborative problem-solving as teams work through challenges during the PIP implementation for their selected quality measure. Participants will learn strategies for identifying and addressing barriers to progress, encouraging open communication, and promoting teamwork. Leadership will play a key role in fostering a problem-solving mindset while keeping the focus on the facility’s PIP goals. Participants will leave with actionable strategies to keep their teams aligned, engaged, and committed to overcoming obstacles throughout the PIP process.

    May 15:  Sustaining Momentum: Leadership Strategies for Continuous Improvements 

    This session will focus on leadership strategies for sustaining momentum in the PIP process and ensuring continuous improvement, even after initial successes. Participants will learn how to lead teams through the ongoing PIP cycle for their selected quality measure, ensuring long-term success and sustained engagement.
